TCM is a time-sensitive and complicated process that has clear deadlines for each step. To help care providers meet those deadlines, we designed pages with clear and simple layouts to help care providers easily prioritize and execute on outstanding tasks.

Step-by-Step Process

Transitional care sees a lot of information, documentation, and user input throughout the process. To help users complete each task without information overload, we segmented the process into core steps the user completes individually, with keeping only relevant information accessible to avoid visual noise.

At any step in the process, care providers are able to revise and update information. Doing so requires them to generate a note that details any changes they made, ensuring any errors can be easily identified and resolved.

Creating a Network

Care providers can have patients in different stages and with different needs for their care. By organizing those patients into their respective facility types, care providers can more accurately maintain varying requirements and deadlines in one unified platform.

Staying Connected

Transitional Care Management is not a one-person task. By providing a messaging platform within the application, providers can more easily coordinate on tasks and share relevant information more quickly from within and across different facilities.
